One of the most comprehensive sets of paper proformas currently available for viewing on line have been created by the Latin American Centre for Perinatology, Women and Reproductive Health (CLAP/WR)  A full description in English including all their proformas is accessible at The Perinatal Clinical Record (Perinatal Information System)
